Filing A Transportation Complaint

If you believe you have been discriminated against by Great Circle in receiving transportation services on the basis of race, color, or national origin, you may file a Title VI complaint.

 

How to file a Title VI complaint about Great Circle:

  1. Use the download PDF link provided to access the Title VI complaint form.
  2. You may file a complaint with Great Circle directly or the Federal Transit Administration, or both.

    To file a complaint with Great Circle – Complete the form and submit via e-mail (This email address is being protected from spambots.     To file a complaint with the Federal Transit Administration – Complete the form and mail to:  Federal Transit Administration, Office of Civil Rights, Region 7, 901 Locust Street, Suite 404, Kansas City, Missouri 64106. Fax to: 816.329.3921. Telephone: 816.329.3920.
  3. Complaints must be filed within 180 days following the date of the alleged discriminatory occurrence and should contain as much detailed information about the alleged discrimination as possible.
  4. Be sure to sign and date the form, and include your contact information.
